We have based our fees on the recommended and typical office fees for uninsured services in Ontario, while taking into account you are paying for an appointment.
Total cost is the form completion and appointment cost (No additional Fees)
- Attending MD/NP Statement: $160
- CPP Disability Form: $125
- CRA Disability Tax Credit: $145
- Drivers Medical: $150
- Form Completion EI Compassionate Care: $70
- Long term or Short-term Disability non ODSP: $150
- Referral Prescriptions (massage, orthotics, chiropractor): $45
- Sick Notes: $45
- School/Camp/Sports Medical Clearance: $75

Narcotic or controlled substances prescriptions (email if you have questions)

Late cancellations incur a fee of 50% of the appointment cost.
